have you been doing every fight? Region 1 and 2 base when you reach an encounter on an invisible 'fights done' counter

If you have been doing all the fights, then it's up to you if when want to make a call on if you think something might have bugged and that you may need to make a report in the bug report forum

I thinks she appears after 28 victorious battles? Though that number could be off depending on how fast you cut threw them and if there are raids going on.

the mirror maze is an entirely different story though lol

the maze is a maze. each room has a colour, and for every room, each of the two mirrors will have a set destination

eg,
Amethyst room, the left mirror goes to the Emerald room, and the right mirror goes to the Sapphire room
Jade's left also goes to the Emerald room
etc.

So you can map it out pretty easily- an "oh, I'm in X room again, I know where Left mirror goes, so I'll take the right this time." kinda thing

There's also two guides pinned in the player guides with a map, one guide specifically for the maze, and the sylestia walkthrough, which obviously covers it as a part of the whole.
