I was wondering what causes a pet to be "accustomed to their name" - not letting the pet be renamed except for purchasing a special item.

It's just an interesting way to keep the game more RP like. It's saying that the pet is too used to its name to get renamed unless you give it the amnesia philter or whatever so it forgets the old name. Basically once you name it you can't rename it unless you buy the philter.

Yeah that kind of bothers me though. Especially when you're a person like me and doesn't pay attention so they make typos.
I think that this is legit if the pet has had its name for over, say, three or more days? Maybe a little more. I know a pet in real life, you wouldn't re-name. But even it isn't used to its name right away (though I might actually put that in the suggestions portion)
But yeah only dia-bought items can re-name them. Which is perfectly fine.

Sorry, was wondering less "why" and more "in what circumstances does this happen". Does it happen any time a pet is named?

Yep. The moment you name a pet, it grows accustomed to the name and you cannot re-name it.
As to why, that's just how it's set up I guess
