From playing different aspects of the game( like nurturing or battling specific enemies)
Some you can even buy from other players

Something like this: https://www.sylestia.com/forums/?thread=83936&page=1. Theyre basically games, activities, and contests setup by the mods. If you place in them (i.e. get 1st, 2nd, 3rd place), youll get a title and one or more prizes. For a title, at least, sometimes simply participating is enough.
